Here you will find useful forms to manage your account or project service need. For your convenience, forms can submitted online, or downloaded, completed and submitted by fax, email, mail, or brought to the District Office for processing.
Tenants: If you are looking to start new service, please contact the property owner as they need to complete the required Owner/Tenant Billing Agreement.

The Closing Account Application provides CHWD with important information, including forwarding address for final bills, dates for when water service is requested to be shut off and more. It also provides the customer with critical information, including potential impacts to water heaters, pools and more if the water is shut off. Property owners wanting to have the District bill their tenants directly for water service must submit this written agreement. This form can only be completed by the property owner. If you are a tenant looking to start new service, please contact the property owner. The owner needs to complete the required Owner/Tenant Billing Application. This form cannot be completed by a tenant.

If the owner agrees to have CHWD bill you directly, your bill will be mailed to you. After you receive the bill you can register for an online account to easily pay and manage your water bill.

Commercial property owners wanting to have the District bill a tenant directly for water service must submit this written agreement. This form can only be completed by the property owner. Most construction projects and activities requiring water from a fire hydrant in the District require a Hydrant Meter Permit, including placement by the District of a construction water meter. A refundable security deposit is required for the use of the meter. In advance of your project, please contact the District to schedule an appointment to determine your water needs. In cases where a fire hydrant is not available or placement of a construction water meter is not possible, a Construction Water Permit may be used to provide water for construction purposes. Projects that require calculation of fire flows for fire district approval may use this Fire Flow Request form to request the District to perform this work using its hydraulic model. Submit online or click here to download the Fire Flow Request.

For the review, comment and approval of private developer and public agency improvement plans. Two sets of preliminary improvement plans shall be provided to the District a minimum of six (6) weeks in advance of the project for initial review.
